Default Excel 2007 text box (ActiveX Control)

I am trying to set up a form on a worksheet for work and need to have a
section for the customer information. I thought that a text box might
worked.Preferrably the activeX Control text box. I want to have it so the
boxes are labeled customer name, address, etc. I have been trying to get this
function to work for a few hours now but I am getting no where. Is there
someone out there who can help me with this issue step by step. I am very
familiar with Excel but just starting using the 2007 version. I really need
to figure this out. Any help would be greatly appreciated.
